Registration and Enrollment Forms

Use the forms below to register, enroll in, or drop Cal State East Bay Continuing Education courses, or to apply for one of our many certificate programs.

Please Note: These forms cannot be used for Open University or regular University courses; or by International Students. Forms can not be accepted by fax.

Courses

Course Registration/Enrollment Form (PDF, 71k)
Complete and submit this form by mail or in person to register and enroll for courses listed in our course catalog.

Once you receive your NetID by email, go online to pay for your courses.

Fees are due at time of enrollment. They can be paid by cash, credit card, electronic check, check by mail (with registration form), or in person. Note: Credit card payments are only accepted online. Failure to pay fees within 48 hours of enrollment could result in being dropped from courses and subjected to a $15 processing fee. 

Course Transfer (PDF, 31k)
To request a transfer from one course to another, complete and return this form or email your program coordinator.

Withdrawal/ Refund (PDF, 89k)
Note: This form is only for use with Continuing Education courses. To withdraw from a regular University course, visit the Enrollment Services forms page and download your form there.

If you cannot continue enrollment in a course for which you have enrolled for a particular quarter, you must officially drop the course online and request a refund or use the form above. Withdrawal from a course requires an official, signed statement, either on this form or in a separate letter. Refund amounts are based upon the date a withdrawal statement is submitted to our offices. Please read our withdrawal/refund policy for more information.

Certificate Programs

Program Application (PDF, 47k)
Some of our programs (paralegal, Chemical Dependency, TESOL, Art Museum and Galery Studies, and Educational Therapy) equire an application and other documentation. To register and enroll in one of these programs, you must complete and return the form above along with other required documentation before registering and enrolling into courses.

Petition for Certificate of Completion (PDF, 33k)
To obtain your official certificate, you must complete all requirements of your program and then complete and submit this form. Upon verification of requirement completion, your certificate will be mailed to you.
